It has been almost a year since they left Serenity, and Simon is beginning to be hopeful. Hopeful, but not stupid, and definitely not careless.
They are taking all the precautions he can think of. They're on a backwoods planet, tiny and forgettable, no economic or cultural worth to speak of. Simon has grown a goatee, and River wears her hair cropped short and dyed black. They're not a brother and sister, a pair of fugitives; they're a doctor and his young wife, moved to the country. River's medication helps her to a point, but Simon has a story to cover the rest of it: rape, torture, the murder of their infant. People are sympathetic, and they don't ask many questions, and they accept the horrors of town as a perfect excuse for why the two of them would move somewhere thousands of miles away from anywhere.
Staying in character was the hardest part, but it has gotten easier as time goes on. Eventually he'll be able to look at the two of them and think of Dr. Gabriel and Regina Matthews, and Simon and River Tam will be gone. And then they'll be safe, forever, and Simon will be able to breathe for the first time since he decoded River's letter all those years ago.
He made the choice of his sacrifices on that day; there hasn't been a time since when he could have gone back. River needs him, and he'll protect her.
He kisses the tears off her eyelids when he moves inside her at night, and the sounds River makes seem almost happy.
